Abstract

In research it is established that at long thyreostatic treatment of a diffuse toxic struma there was no involution of a hypertrophy of a myocardium and a dilatation of cavities of heart. Tendencies to augmentation of mass of a myocardium of a left ventricle were observed at all patients at continuous thyreostatic therapy. In development of thyrocardiac changes of a myocardium an important role was played by the disturbances which have happened prior to the beginning of treatment, and also accompanying arterial hypertension. In patients with arterial hypertension observed a significant increase in the index of left ventricular mass, an increase in cavities of the left ventricle and left atrium. Arterial hypertension (AH) is one of the most frequent manifestations of thyrotoxicosis. Its development is associated with increased cardiac output due to the direct effects of excess thyroid hormone (TH) on the myocardium, the reflex response to increased metabolic demands, activation of the renin-angiotensin system and an increase in blood volume. [1] In 1899 R.Kraus coined the term "goiter heart", which is understood as a symptom of disorders of the cardiovascular system caused by the toxic effects of excess thyroid hormones, characterized by the development of hyperthyroidism, thyroid, malnutrition, infarction, and congestive heart failure [2]. At the heart of the heart of hyperthyroidism is an increase in myocardial contractility. But for a long time still debated question of the direct effect of thyroid hormones on the myocardium or indirectly via catecholamines [3]. Analogs of adrenaline and noradrenaline, formed of thyroid hormones may function as psevdokateholaminy interacting with b-adrenoceptors [4]. It is believed that the elimination of hyperthyroidism helps eliminate this symptom of the disease. However, studies have long-term results of treatment of diffuse toxic goiter (DTG) revealed excess risk of death from hypertension in these patients is 1.2 times of severe arrhythmias - by 1.5 times, from heart attacks and strokes, respectively - 1.1 and 1 , 4 times. [5] The purpose of the study - the study of the role of hypertension in the event of changes in thyrotoxic heart and reverse development against the background of long-term drug therapy. Material and methods. The study involved 62 patients with Graves 'disease after a long thyrostatic diagnosis of Graves' disease therapy was verified by clinical relevance of these changes in triglycerides (TTG T4sv, T3sv), ultrasonography of the thyroid gland.
